Facility Inspections

Grade: B-minus

Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection scores weigh a host of factors included in a facility's inspection report. One of the most important criteria we look at is the quantity and severity of deficiencies. Although this facility had some deficiencies on its report, none of them were serious based on CMS' deficiency scale. Lastly, this facility was flagged by CMS for possible abuse or neglect. We prefer to avoid facilities with this designation. Please do your best to find an alternative nursing home.

Long-term Care Quality

Grade: B-minus

Remarkably, we gave this facility a grade of B- for our long-term care grade, which isn't too bad of a score. In crafting these long-term care ratings, we look at the nursing home's ability to maintain its permanent residents' health and quality of life. After assessing the quantity of care provided by aids and other staff, we turned to the nursing home's vaccination data. We were optimistic to discover that this nursing home vaccinated 100 percent of its residents for pneumonia. This facility keeps its residents out of the hospital. It had less than one hospitalization per one thousand long-term resident days, which is a remarkably low figure.

Short-term Care Quality

Grade: D

This nursing home's second lowest score is short-term care, where it received just a D in that area. This is a well below average grade. In the area of short-term care, we strive to qualify measures of a facility's rehabilitation. We assess a facility's skilled nursing services, such as those performed by registered nurses and physical therapists. Given its grade in this area, we were not surprised to learn that this nursing home is well below average in terms of the number of physical therapy and registered nurse hours offered to its residents based on the data we assessed. Lastly, we considered the percentage of patients who returned home from this nursing home. This facility didn't fare well here either. In fact, we found that just 0 percent of this facility's patients were able to return home. This figure was well off the national average.

Moving on to our next area, this nursing home really did not fare well in this area either. With a bottom of the barrel grade of F in nursing, this is just about as bad as it gets. Our nursing score looks at the nursing home's nurse staffing levels. We consider both the levels of licensure of the nurses as well as the amount of time spent with residents. This nursing home provides 0.3 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. In addition to being below average in nursing hours per resident, this place also had poor marks in the quality-based measures we looked at in calculating our nursing scores. We looked at the percent of residents sustaining major falls and pressure ulcers. This place was at roughly 150 percent of the national average in both of these metrics. This is likely a bad sign when you consider that many falls and bed sores are preventable with better nursing care. These statistics pulled down this facility's nursing rating significantly.
