Facility Inspections
While this place's overall grade was as bad as it gets, it actually didn't perform terribly in the area of inspections. In fact, we awarded it a B- for that category, which is one of our better scores. Inspection scores weigh several factors included in a facility's inspection reports. One key criteria we rely on is the quantity and severity of deficiencies. Fortunately, although this facility had a few minor dings on its report, it had no severe deficiencies. Severe deficiencies are found in categories G, H, I, J, K and L. This means that CMS did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple minor deficiencies are not the end of the world.
Nurse Quality
Unfortunately, this facility only received a grade of D for its nursing grade, which was actually one of its better category grades. Nursing grades are primarily associated with the facility's nurse staffing. This place averaged a meager 0.3 hours of nursing care per patient on a daily basis. This statistic is well below average. Finally, we also assessed some nursing quality-based metrics in determining our nursing scores. This place performed well in the area of minimizing its residents' pressure ulcers and major falls.

Short-term Care Quality
Our third area we graded is short-term care, where this nursing home was given an F. Short-term care ratings are based in part on a facility's quantity of highly-skilled skilled healthcare professionals. This includes a wide spectrum of nursing services, spanning from registered nurses to physical and respiratory therapists, as well as other forms of therapy. Based on its grade in this category, we weren't surprised to learn that this nursing home was well below average in terms of the number of registered nurse and physical therapy hours offered to its patients based on the data we looked at. Finally, we considered the percentage of patients who were able to return to the community from this nursing home. We found that just 0 percent of this nursing home's residents returned home.
Long-term Care Quality
The final category we assessed was long-term care, in which this nursing home was given a rock bottom score here as well. Sadly, it received an F for this category, which is obviously a major concern. For prospective residents seeking a permanent residence as opposed to rehabilitation, long-term care grades are an important measure. After we finished assessing the volume of care provided by nurses, we next considered the facility's vaccination record. We were optimistic to learn that this facility vaccinated 100 percent of its residents for pneumonia. Surprisingly, this place also fared well at keeping its residents out of the hospital. It had only 0.01 hospitalizations per 1,000 long-term resident days. Unfortunately, a few of its other scores in this category weren't as favorable as these.
