Facility Inspections
While we were fairly low on this facility overall, we awarded it an A+ for our inspections rating. This score is far more impressive than the facility's overall grade. Inspection grades are based on pieces of information located in the a nursing home's recent inspection reports. Deficiencies are an important item to look for on these inspections. Most importantly, you should avoid facilities that have severe deficiencies associated with endangerment of patients. This particular nursing home had just a single deficiency on its inspection report and it was not one deemed to pose a threat to patient health or safety. Despite this deficiency, this was still a relatively positive inspection report.
Long-term Care Quality
Unfortunately, this facility only earned a D for its long-term care score, which happens to be one of its higher category grades. Long-term care scores of this caliber generally are a bad sign for the quantity and quality of care provided by nurses and aids. On top of looking at the volume of care provided by aids and other staff, we also looked at the percentage of residents vaccinated for pneumonia. This facility administered the vaccine to 91.03943 percent of its residents, which is a few points less than we anticipated. To our surprise, this facility actually fared well at keeping its residents out of the hospital. Indeed, it had just 0.02 hospitalizations per 1,000 long-term resident days. This is its best feature in this category.

Short-term Care Quality
In our next category, we gave this nursing home a rock-bottom F in the category of short-term care. In the area of short-term care, we attempt to assess indicators of a facility's rehabilitation services. We look at a nursing home's skilled nursing services, including those performed by registered nurses and physical, occupational and other therapists. Considering its score in this category, we were not surprised to find that this place was well below average in terms of the quantity of physical therapy and registered nurse hours offered to its residents based on the measures we assessed. Frankly, this was what we expected to find. Lastly, we assessed the percentage of residents who were able to return to the community from this nursing home. We found that just 4.7 percent of this facility's patients returned home.
Nurse Quality
The final category we assessed is nursing. Unfortunately, we gave it an abysmal F for this area, which is a bottom of the barrel grade. We scrutinized the skill-level of nurses working for the facility, in addition to the number of hours those nurses were with patients, in determining our grade in this category. This facility provided just 0.3 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. This facility also had terrible marks in some of the quality-based measures to pair with its low nursing hours. We looked at the percent of patients experiencing pressure ulcers and we were quite disappointed. This nursing home was at roughly 1.5 times the national average in this area. This is likely a bad sign in light of the fact that so many pressure ulcers are preventable with better nursing care. This metric helped pull down this nursing home's nursing score significantly.
