In addition to performing well in its overall grade, this facility performed at an even higher level in the area of inspections. In fact, it received a nearly flawless inspection report this year. As a result, it received one of our highest grades in that category with a score of A+. Our inspection scores weigh several factors found on a nursing home's inspection reports. One of the most important criteria we weigh heavily is the quantity and severity of deficiencies. Places with higher grades in this area tend to have very few of these severe deficiencies. This place was hit with 2 deficiencies on its inspection report, but fortunately none were considered to be major deficiencies. This means that the inspectors did not consider any of these deficiencies to create an immediate risk to resident health or safety. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

Among the reasons we graded this facility so favorably is that it was awarded a strong long-term care score. Indeed, long-term care turned out to be its second most favorable category score. In that category, we awarded this nursing home a grade of A. When facilities receive this type of grade in this category it is a good sign for patient care and indicates that the facility is well-staffed with nurses aids. In addition to considering the quantity of care provided by nurses aids and other staff, we also looked at the percentage of residents vaccinated against pneumonia. This nursing home provided the vaccine to 100 percent of its patients. Vaccines are critical to keeping residents healthy. Finally, this facility was able to limit hospitalizations. It had only 0.02 hospitalizations per 1,000 long-term resident days, which is a remarkably low number.

Next, we looked at the nursing category where we awarded this nursing home a grade of C. This facility didn't perform quite as well in this category as it did in some other categories. Nevertheless, this is still a respectable score. When determining our nursing scores, we analyze both the number of hours nurses spend with residents and the skill levels of those nurses. This particular facility provided just 0.3 hours of nursing care per patient each day. This figure is far below the national average. Lastly, despite not having the highest total nursing hours, this facility actually performed admirably in the area of preventing major falls. This is usually a good indicator that a place has sufficient quality controls in place.

Switching gears to our final category, this facility really did not fare well. Due to a bottom of the barrel grade of F in that category, this is notably its weakest area. In computing our short-term care grades, we quantify the facility's skilled nursing services, such as those performed by registered nurses, occupational therapists, physical therapists and other types of skilled professionals. Finally, we considered the percentage of residents who were able to return to the community from this facility. This definitely wasn't a strength for this facility. We found that just 4.4 percent of this nursing home's residents returned home. At most facilities, around half of their patients are able to return home so this is well below average.
