Facility Inspections
B-
Historical
While we graded this facility incredibly poorly overall, it received a fairly respectable inspection report this year. As a result, it received a decent grade in that category with a score of B-. We believe that one of the most important factors in assessing an inspection report is deficiencies. In this regard, we especially focus on the severity of the deficiencies. While this place had some minor dings on its report, it had no severe deficiencies. Severe deficiencies are those labeled as categories G, H, I, J, K and L. This means that the government inspectors didn't consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Finally, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.
Short-term Care Quality
C
Historical
We were also stunned to learn that this facility earned a decent short-term care rating in our assessment. We gave it a grade of C in this category. With our short-term care score, we try to forge a fair barometer for rehabilitation services. In this process, we analyze the nursing home's levels of skilled nursing services, including both registered nurses and physical therapy, as well as speech therapy. Remarkably, this facility provides its residents with at least 50% more physical therapist hours per week than the average facility. Finally, we looked at the percentage of patients who were able to return to the community from this facility. This place outperformed the majority of facilities in the country with 50.8 percent of its patients returning home.

Long-term Care Quality
D
Historical
We awarded this nursing home a grade of just D for our long-term care grade. This is not an impressive grade. For patients in need of a permanent residence rather than rehabilitation, long-term care is a very important category. Once we looked at the quantity of care provided by aids and other staff, we then analyzed the facility's vaccination records. We were pleased to discover that this nursing home vaccinated 98.12734 percent of its residents for pneumonia. To our surprise, this facility was able to limit hospitalizations. With just 1.43 hospitalizations per one thousand long-term resident days, this facility has less hospitalizations than the majority of nursing homes. Unfortunately, a few of its other scores in the category were not as favorable as these.
Nurse Quality
D
Historical
The next category we rated was nursing, was this nursing home's weakest area. We gave this nursing home a grade of D in that area. Our nursing rating consists of many factors, however, the paramount one is the quantity of nurse hours per patient per week. This nursing home provides 3.4 hours of nursing care per resident per day. This is slightly below average. Lastly, we also looked at some quality measures in calculating our nursing ratings. This facility was above average in two of the data points we focus on, with solid marks for minimizing its residents' pressure ulcers and major falls.
