Facility Inspections
While we were not high on this place overall, we awarded it a B+ for our inspections category. This grade is far better than the facility's overall grade. Perhaps the most significant factor we look at in determining our inspection grades is deficiencies. Deficiencies are found on a facility's recent inspection reports. It is generally in your best interest to avoid nursing homes that have a long list of deficiencies. Fortunately, although this nursing home had some minor dings on its government inspection report, it had no severe deficiencies. Severe deficiencies are found in categories G, H, I, J, K and L. This tells you that the inspectors didn't consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. A few minor deficiencies are not the end of the world.
Short-term Care Quality
This nursing home's next best category is short-term care. We gave it a grade of C in this area. This is essentially a middle of the pack score in this category. In the category of short-term care, we seek to qualify indicators of a nursing home's rehabilitation services. We analyze the nursing home's skilled nursing services, including those performed by registered nurses and physical and occupational therapists. Unfortunately, this place was below average in terms of its level of physical therapy and registered nurse hours provided to its residents based on the metrics we assessed. Finally, we considered the percentage of patients who eventually returned home from this nursing home. We found that 5.3 percent of this facility's patients were able to return home. This figure was well below the national average.

Long-term Care Quality
Turning to the area of long-term care, this facility was given just a D in this area. For long-term care residents, the facility's primary goal is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate residents. After we finished looking at the volume of care provided by nurses, we turned to the facility's vaccination data. We were optimistic to learn that this nursing home vaccinated 95.69893 percent of its residents for pneumonia. To our surprise, this facility was able to limit hospitalizations. With just 0.02 hospitalizations per 1,000 long-term resident days, this nursing home has fewer hospitalizations than most nursing homes. Unfortunately, some of its other scores in the category were not as favorable as these.
Nurse Quality
The next area we assessed is nursing, where this nursing home was given a bottom of the barrel score here also. It received an abysmal F in this area, which is obviously somewhat alarming. Nursing grades are based in large part on nurse staffing levels. With only 0.4 hours of nursing care per patient on a daily basis, this nursing home's nurse staffing levels were well below the national average. Lastly, we also looked at some nursing quality-based metrics in computing our nursing grades. This nursing home performed well in terms of avoiding pressure ulcers and major falls.
