While this facility's overall grade was decent, it really fared well in the category of inspections. In fact, we awarded it a grade of A for that category, which is one of our best scores. These inspection ratings weigh several factors, including deficiencies, substantiated complaints and federal fines. You can find more information about each of these factors by reviewing copies of nursing homes' inspection reports. While this place had a few deficiencies on its government inspection report, none of them were serious based on CMS' scale. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

Turning to the category of long-term care, we awarded this nursing home a score of A in that category. When nursing homes receive a score in this range in this category it typically means it has plenty of staff and is an overall good place to live on a permanent basis. After we finished looking at the amount of nursing care, we turned to the nursing home's vaccination records. This nursing home vaccinated 97.37609 percent of its residents against pneumonia, which is very impressive. Pneumonia sadly is often a dangerous health condition for nursing home patients so we prefer when a facility does not take any chances. The last datapoint we assessed was its hospitalization rate. We found that this place had 1.83 hospitalizations per one thousand long-term resident days, which is middle of the road in this area.

Turning to another strength for this facility, it also received an elite nursing rating. We gave this facility a score of D. The nursing score analyzes several factors, but the most important consideration is the quantity of nurse hours per patient per week. This facility provided 3.6 hours of nursing care per resident per day. This is below the national average. In addition to its subpar nursing hour totals, this place received poor marks in several of the quality-based measures we considered in calculating our nursing scores. We found that a higher percentage of this nursing home's patients suffered falls leading to serious injury. We believe that many falls could be prevented with better nursing care. This statistic contributed to this place receiving a poor nursing score.

The next area we graded was short-term care. Unfortunately, we gave it an abysmal F for this area, which is an abysmal grade. This is obviously a significant concern. In forming these short-term care ratings, we quantify the nursing home's skilled nursing services, including those performed by registered nurses, speech therapists, physical therapists and other types of skilled professionals. Our objective is to create a barometer for sizing up the rehabilitation services of nursing homes. We found that this facility was below average in several staffing areas we assessed. In fact, it offered fewer registered nurse and physical therapist hours per patient than most facilities. Lastly, we considered the percentage of residents who were able to return to the community from this facility. We found that just 36.8 percent of this nursing home's patients returned home.
