Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a grade of C for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection grades weigh several factors, including deficiencies and federal fines. You can find more information about each of these factors by obtaining copies of nursing homes' inspection reports. This facility had 2 deficiencies on its government inspection report, but none were category G through L deficiencies. This indicates the government inspectors did not deem any of the deficiencies to present an imminent danger to resident safety or health. Unfortunately, we also need to draw your attention to the fact that this facility received significant government fines in recent years.
Long-term Care Quality
Sadly, this nursing home's second most respectable category is long-term care. As it turns out, with a grade of D in this area, it still performed relatively poorly. When facilities receive a grade in this range in this category it generally means the nursing home didn't perform well in our measures relating to resident care. In addition to assessing the volume of care provided by nurses aids and other staff, we looked at the number of residents vaccinated against pneumonia. This facility provided the vaccine to 61.153843 percent of its patients. This is multiple points less than what we expected. To our surprise, this place actually fared well at keeping its residents out of the hospital. Indeed, it had just 0.03 hospitalizations per one thousand long-term resident days. This was its best feature in this category.

Nurse Quality
This facility's next best category was nursing, but even there it received a grade of just D. This is still a well below average grade. In computing a facility's nursing score, we weigh the number of hours nurses spend with patients as well as the level of skill of those nurses. This facility provided just 0.3 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. Finally, we also assessed some nursing quality measures in determining our nursing grades. This facility fared well in terms of minimizing its residents' major falls and pressure ulcers.
Short-term Care Quality
Moving on to the next area, this nursing home really didn't perform well in this category either. Due to a very poor grade of F in short-term care, this is about as terrible as it gets. In the area of short-term care, we seek to evaluate indicators of a nursing home's rehabilitation services. We analyze a facility's skilled nursing services, including those performed by registered nurses and physical and occupational therapists. With its grade in this category, we weren't surprised to discover that this facility is well below average in terms of its number of registered nurse and physical therapy hours provided to its residents based on the data we assessed. Finally, we considered the number of residents who who were able to eventually return home from this nursing home. This facility didn't fare well here either. In fact, we found that just 3.4 percent of this facility's residents were able to return home. This figure was well off the national average.
