Facility Inspections
Although we gave this facility a terrible overall grade, we awarded it a grade of B- for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection grades weigh several factors, including deficiencies and federal fines. You can find more information about each of these factors by obtaining copies of nursing homes' inspection reports. While this nursing home had some minor dings on its report, it had zero severe deficiencies. Severe deficiencies are those labeled as categories G, H, I, J, K and L. This means that the inspectors didn't consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple minor deficiencies aren't the end of the world.
Long-term Care Quality
Sadly, this facility received an F for its long-term care grade. Facilities that do not fare well in long-term care typically aren't as well-staffed and are lagging in a few of the areas of routine healthcare services we assessed. On top of looking at the amount of care provided by nurses aids and other staff, we also looked at the number of residents vaccinated against pneumonia. This nursing home provided the vaccine to a relatively low 81.14035 percent of its residents, which is several points lower than we expected. To our surprise, this place was actually decent at keeping its patients out of the hospital. Indeed, it had just 0.02 hospitalizations per one thousand long-term resident days. This was its best feature in this category.

Nurse Quality
We also wanted to draw your attention to this facility's poor nursing grade where it received an F. Nursing ratings are based in large part on nurse staffing levels. With only 0.3 hours of nursing care per resident on a daily basis, this facility's nurse staffing levels were quite a bit below the national average. Finally, we also looked at some nursing quality measures in calculating our nursing grades. This nursing home fared well when it comes to minimizing its residents' major falls and pressure ulcers.
Short-term Care Quality
The next category we scored was short-term care, in which this facility received a very poor score in this area as well. We gave it a lowly F for this category, which is definitely a major disappointment. Our short-term care grades are probably more meaningful for individuals in need of rehabilitation services. Rehabilitation usually mandates higher levels of skilled nursing services. This means not just nursing services, but also physical and occupational therapy, as well as other forms of therapy. With its grade in this category, we weren't surprised to discover that this facility is well below average in terms of its number of registered nurse and physical therapy hours provided to its residents based on the data we assessed. Finally, we considered the number of residents who who were able to eventually return home from this nursing home. This facility didn't fare well here either. In fact, we found that just 3.4 percent of this facility's residents were able to return home.
