Facility Inspections
While this place's overall score was terrible, it actually fared well in the category of inspections. In fact, we awarded it a B+ for that area, which is one of our better scores. Inspection grades take several factors into consideration, including deficiencies and federal fines. You can find more information about each of these items by obtaining copies of nursing homes' inspection reports. This place was assessed 2 deficiencies on its inspection report, but fortunately none were considered to be major deficiencies. This indicates that the inspectors didn't deem any of these deficiencies an immediate threat to patient health or safety. A few minor deficiencies are not necessarily the end of the world.
Long-term Care Quality
Sadly, this facility only earned a D for its long-term care grade, which ended up being one of its better category scores. When facilities receive a grade in this range in this category it generally means the facility did not perform well in our measures relating to patient care. One of the statistics we considered in addition to nurse's aid hours was vaccines. We were a bit bothered by the fact that this facility administered the pneumonia vaccine to just 72.96296 percent of its patients. Surprisingly, this place was actually decent at keeping its patients out of the hospital. It had just 0.04 hospitalizations per 1,000 long-term resident days. This was its best score in this category.

Nurse Quality
We also wanted to point out that this nursing home received an F for its nursing grade. The nursing grade considers many datapoints. The most important factor is the number of hours nurses spend with patients. This nursing home offered only 0.3 hours of nursing care per resident each day. This is an alarmingly low total. Lastly, we also looked at some nursing quality measures in computing our nursing scores. This place fared well in terms of avoiding pressure ulcers and major falls. We consider these datapoints to be reliable measures of the quality of nursing care.
Short-term Care Quality
The last category we assessed is short-term care. Unfortunately, we gave it a lowly F for this area, which is an abysmal grade. In computing our short-term care scores, we look at the facility's levels of highly skilled nursing, which includes registered nurses, physical therapists speech therapists and other types of therapists. This area is more often than not a fair measure of the facility's rehabilitation. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. The last measure we looked at in this category is the percentage of residents who were able to leave the nursing home and return to the community. We discovered that just 3.4 percent of this facility's patients were able to return home, which is well below average. The combination of these poor metrics was damning for this facility's short-term care score.
