Facility Inspections
This nursing home received a solid overall grade as described above, but we really wanted to point out its nearly flawless health inspections in recent years. We gave them an impressive grade of A+ in this category. Perhaps the most significant factor we look at in computing our inspection grades is deficiencies. These deficiencies can be found on a facility's inspection reports. Places with higher scores in this category typically dodged the more severe deficiencies involving things like patient abuse. Fortunately, although this nursing home had a few minor dings on its government inspection report, it had no severe deficiencies. Severe deficiencies are those labeled as categories G, H, I, J, K and L. This tells you that the inspectors did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
Long-term Care Quality
Turning to the category of long-term care, we gave this facility a score of A in that category. Nursing homes that do well in long-term care typically are well-staffed and provide extensive hands on care to residents. In addition to looking at the amount of care provided by nurses and other staff, we analyzed the number of residents vaccinated against pneumonia. This facility gave the vaccine to 100 percent of its residents, which is much higher than most nursing homes. Clearly, this facility is doing something right in this area as it also excelled at keeping its residents out of the hospital. It had only 0.02 hospitalizations per one thousand long-term resident days, which is a remarkably low figure.

Nurse Quality
Moving on to another strength for this facility, it also earned an elite nursing rating. In fact, we gave this nursing home an impressive grade of D. Our nursing score looks at the nursing home's level of nurse staffing. We weigh both the levels of training of the nurses and the number of hours spent with patients. With only 0.4 hours of nursing care per patient per day, this nursing home's nurse staffing levels were far below average. Finally, we also looked at some nursing quality-based metrics in calculating our nursing scores. This place fared well in terms of minimizing its residents' pressure ulcers and major falls.
Short-term Care Quality
The final area we graded was short-term care. Sadly, it received a lowly F in this category, which is a poor grade. This is clearly a significant disappointment. Short-term care scores are based in part on the nursing home's quantity of highly-skilled nursing services. This includes a vast scope of nursing services, ranging from registered nurses to physical and occupational therapists, not to mention other types of therapy. It appears that this facility was subpar in multiple staffing metrics we assessed. The facility provided fewer physical therapist and registered nurse hours per patient than most nursing homes. The final statistic we considered in this category is the percentage of patients who were able to leave the facility and return to the community. We discovered that just 4.8 percent of this facility's residents were able to return home, which is well below average. The combination of these poor metrics doomed this nursing home's short-term care score.
