We gave this facility just a D for our short-term care grade. This is not a score to write home about. In our short-term care assessment, we try to forge a valuable barometer for rehabilitation. In doing so, we analyze a nursing home's levels of highly-skilled nursing services, including both registered nurses and physical therapy, as well as speech therapy. Given its score in this area, we were not stunned to find that this nursing home was well below average in terms of its number of physical therapy and registered nurse hours offered to its patients based on the metrics we assessed. Finally, we considered the number of residents who ultimately were able to return home from this facility. This facility didn't fare well here either. In fact, we found that just 4.6 percent of this facility's patients were able to return home. This figure was quite a bit off the national average.
