Facility Inspections
While this nursing home's overall grade was as bad as it gets, it actually didn't perform terribly in the area of inspections. In fact, we awarded it a grade of C for that category, which is one of our better scores. Inspection ratings weigh a host of factors included in a nursing home's inspection reports. One key criteria we weigh heavily is the number and severity of deficiencies. Surprisingly, this place was assessed a category J through K deficiency, which are some of the worst possible categories of deficiencies. This typically tells you that the government inspectors found a deficiency which placed patient health or safety in imminent danger. In addition to having severe deficiencies, this nursing home was cited by CMS for possible abuse or neglect. This is generally a really bad sign. We would never recommend this facility.
Long-term Care Quality
We awarded this nursing home a grade of just D for our long-term care rating and that actually turned out to be its next most respectable category. If you are looking for services other than short-term rehabilitation, you should take a close look at each facility's long-term care ratings. After considering the amount of care provided by nurses and other staff, we turned to the nursing home's vaccination record. We were pleased to discover that this nursing home vaccinated 100 percent of its residents for pneumonia. Surprisingly, this facility was able to keep its residents out of the hospital. Indeed, it had only 0.02 hospitalizations per one thousand long-term resident days. Unfortunately, a few of its other scores in the category weren't as favorable as these.

Short-term Care Quality
This nursing home's second least favorable category was short-term care, where it received a grade of just D in that area. This is a relatively poor grade. Short-term care ratings are critical for residents requiring rehabilitation. Rehabilitation typically utilizes more skilled nursing services. This means a vast range of nursing services, spanning from registered nurses to physical and respiratory therapists, as well as other types of therapy. With a score in this range in this category, we weren't shocked to discover this nursing home was well below average in the two key staffing metrics we looked at. The nursing home supplied far fewer registered nurse and physical therapist hours per resident than most other facilities. Finally, we looked at the number of residents who eventually returned home from this facility. This nursing home didn't fare well here either. In fact, we found that just 0 percent of this facility's residents were able to return home.
Nurse Quality
Turning to the next category, this nursing home really didn't perform well in this category either. With a poor grade of F in nursing, this is about as as uninspiring as it gets. The nursing grade features numerous factors. The most important factor is the quantity of hours nurses spent with patients. This particular facility provided just 0.2 hours of nursing care per resident on a daily basis. This statistic is far below the national average. On top of its subpar nursing hour totals, this place earned suboptimal scores in some of the quality-based metrics we looked at in determining our nursing ratings. We found that more of this facility's patients suffered falls leading to major injury. We would argue that many falls could likely be prevented with better nursing care. This metric contributed to this facility receiving a poor nursing grade.
