Facility Inspections
While we graded this nursing home incredibly poorly overall, it actually received a relatively impressive inspection report this year. As a result, it received one of our more favorable grades in that area with a B+. We take several aspects of a facility's inspection report into consideration in calculating these inspection grades. One critical factor is health deficiencies. You should keep in mind that the severity of the deficiencies is usually more important than the number of deficiencies, as some can be relatively minor. Although this facility had a few deficiencies on its government inspection report, none of them were serious based on CMS' deficiency scale. A couple minor deficiencies are not the end of the world.
Nurse Quality
We gave this facility a grade of just D for our nursing rating and that actually turned out to be its second highest score. Our nursing rating focuses on the nursing home's level of nurse staffing. We factor in both the levels of training of the nurses as well as the amount of time spent with patients. This place provided only 0.3 hours of nursing care per resident each day. Finally, we also assessed some nursing quality-based metrics in determining our nursing scores. This place performed well in the area of avoiding pressure ulcers and major falls. Many consider these metrics to be good measures of the quality of nursing care.

Short-term Care Quality
This nursing home's second worse score was short-term care, where it earned a grade of just D in that area. This is a relatively poor grade. In the category of short-term care, we try to assess indicators of a nursing home's rehabilitation. We analyze the facility's skilled nursing services, such as those performed by registered nurses and physical, occupational and other therapists. With its score in this area, we weren't stunned to find that this place is well below average in terms of the quantity of registered nurse and physical therapy hours provided to its residents based on the figures we looked at. The last statistic we assessed in this category is the percentage of residents who were able to leave the nursing home and return to the community. We discovered that just 5 percent of this facility's patients were able to return home, which is well below average. The combination of these poor metrics doomed this facility's short-term care score.
Long-term Care Quality
The final category we assessed is long-term care. Sadly, it received an abysmal F for this category, which is a very poor grade. Facilities that don't fare well in long-term care often do not provide as much nursing care and also may be struggling in a few of the areas of routine healthcare services we assessed. Once we looked at the volume of care provided by nurses aids and other staff, we turned to the nursing home's vaccination records. We were a bit bothered by the fact that this facility vaccinated 85.2459 percent of its residents against pneumonia, which happens to be quite a bit below what we expected. To our surprise, this place was actually able to limit hospitalizations. With only 0.02 hospitalizations per 1,000 long-term resident days, this facility had less hospitalizations than many nursing homes. This was its best score in this category.
