Long-term Care Quality
We were not high on this place's overall grades or its category grades. We gave it a D for long-term care and that actually turned out to be its highest grade. If you are looking for services other than rehabilitation, you should take a close look at each nursing home's long-term care grades. After looking at the quantity of care provided by nurses and other staff, we then analyzed the facility's vaccination records. We were pleased to find that this facility vaccinated 97.89156 percent of its residents for pneumonia. To our surprise, this facility was able to limit hospitalizations. With only 0.03 hospitalizations per 1,000 long-term resident days, this nursing home has less hospitalizations than most nursing homes. Sadly, some of its other scores in this category weren't as favorable as these.
Short-term Care Quality
Unfortunately, we gave this facility an F for its short-term care grade. Short-term care scores are based in part on the nursing home's quantity of skilled nursing services. This means a wide range of services, spanning from registered nurses to physical and occupational therapists, in addition to other variations of therapy. Not surprisingly, we discovered that this nursing home provides substantially less physical therapist and registered nurse hours per resident than the average facility. Finally, we assessed the number of residents that ultimately returned home from this facility. This facility didn't fare well here either. In fact, we found that just 4.7 percent of this facility's residents were able to return home.

Nurse Quality
We also wanted to draw your attention to this facility's poor nursing grade where it received an F. Nursing grades are based in large part on nurse staffing levels. This facility provided just 0.4 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. Finally, we also looked at some nursing quality-based metrics in calculating our nursing grades. This facility fared well in the area of minimizing its residents' pressure ulcers and major falls.
Facility Inspections
Turning our final category of inspections, this facility received an abysmal grade. When a nursing home has this bad of an inspection rating, you should look for severe deficiencies on its inspection report. Severe deficiencies indicate a risk to patient safety. This particular nursing home was assessed 3 deficiencies on its inspection report. The only favorable thing we found is that none of the deficiencies were in the categories that suggest they created a risk to resident health or safety. Finally, CMS cited this nursing home for possible abuse or neglect. Hopefully, its performance improves in the future, but until then we simply can't recommend this facility.
