Facility Inspections
The primary reason this turned out to be a decent nursing home is that it earned an excellent inspection grade. In fact, its inspection grade was far better than its overall grade. In the inspections category, we gave this facility an A+. Inspection ratings are based on many items found on the a nursing home's recent inspection reports. Facilities that excel in this area tend to have very few deficiencies on their reports. Most importantly, these places generally do not have any severe deficiencies which are associated with risks to patient safety. Although this nursing home had a few deficiencies on its report, none of them were severe based on CMS' scale. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
Long-term Care Quality
This nursing home also excelled in the category of long-term care, where we awarded it a score of A-. Very few nursing homes fared better in this category. In a long-term care setting, the facility's primary goal is to keep patients as healthy and safe as possible. This is different than skilled nursing or short-term care where the goal is to rehabilitate patients. In addition to considering the quantity of care provided by nurses and other staff, we analyzed the percentage of residents vaccinated for pneumonia. This nursing home vaccinated 87.53623 percent of its patients. This is somewhat below what we were hoping for. Clearly, this place is doing something right in this area as it also excelled at keeping its residents out of the hospital. It had less than one hospitalization per one thousand long-term resident days, which is a remarkably low figure.

Short-term Care Quality
Turning to the area of short-term care, this nursing home was awarded a grade of just D in this category. In the area of short-term care, we seek to evaluate measures of a facility's rehabilitation. We assess the facility's skilled nursing services, including those performed by registered nurses and physical therapists. Unfortunately, this nursing home was below average in terms of its quantity of physical therapy and registered nurse hours provided to its residents based on the measures we assessed. Lastly, we considered the percentage of residents who were able to return to the community from this facility. We found that just 4.7 percent of this nursing home's patients returned home.
Nurse Quality
Moving on to the next category, this facility really did not fare well. With a rock bottom grade of F in nursing, this was this facility's poorest category. When calculating our nursing ratings, we factor in both nursing hours and the levels of licensure of the nurses. This nursing home offered a meager 0.3 hours of nursing care per resident per day. In addition to its subpar nursing hour totals, this place received suboptimal scores in some of the quality-based measures we considered in computing our nursing grades. We found that a higher percentage of this facility's residents experienced falls which led to major injury. We believe that many falls could be prevented with better nursing care. This metric contributed to this place receiving a terrible nursing grade.
