Nurse Quality
Unfortunately, we didn't have many positive things to say about this nursing home's overall score or its category grades. Its best category came in nursing. Nevertheless, with a D in this category, it still performed relatively badly. Nursing grades are tied to levels of nurse staffing. This nursing home provided 0.3 hours of nursing care per patient on a daily basis. This is below the national average. Lastly, we also looked at some nursing quality-based metrics in calculating our nursing scores. This facility fared well in the area of avoiding major falls and pressure ulcers.
Facility Inspections
Among this facility's alarming grades was an F for its inspection score. More often than not, this means we found some red flags on the facility's inspection reports. With places that received this poor of an inspection score, we would scrutinize any severe deficiencies on its inspection report. This nursing home had 8 deficiencies by CMS. The only positive thing we found is that none of its deficiencies were in the categories that indicate they created a risk to resident health or safety. Lastly, we also want to draw your attention to the fact that this place was assessed large government fines totaling more than $100,000. This often signifies that a facility'spoor track record spanned more than one year.

Long-term Care Quality
This facility ended up receiving a an abysmal grade in our long-term care area. It received an F in this category. If you are seeking services other than short-term rehabilitation, you should take a close look at long-term care scores. After assessing the quantity of care provided by nurses aids and other staff, we then analyzed the facility's vaccination records. We were optimistic to find that this facility vaccinated 100 percent of its residents against pneumonia. Surprisingly, this place was able to limit hospitalizations. With just 0.01 hospitalizations per 1,000 long-term resident days, this nursing home had fewer hospitalizations than many nursing homes. Unfortunately, some of its other scores in the category were not as favorable as these.
Short-term Care Quality
Moving on to the next category, this nursing home did not fare very well in this area either. With an abysmal grade of F in short-term care, this is just about as bad as it gets. Short-term care ratings are important for prospective residents requiring rehabilitation. Rehabilitation typically requires more skilled nursing services. Skilled nursing includes a broad range of services, ranging from registered nurses to physical and speech therapists, as well as other forms of therapy. With a score of this caliber in this category, we were not surprised to find that this place was below average in two of the key staffing metrics we looked at. In fact, it provided significantly fewer physical therapist and registered nurse hours per patient than most facilities. Lastly, we looked at the percentage of residents who who were able to eventually return home from this facility. This facility didn't fare well here either. In fact, we found that just 3.3 percent of this facility's patients were able to return home. This figure was quite a bit off the national average.
