While this facility's overall score was as bad as it gets, it performed well in the area of inspections. In fact, we awarded it a grade of B for that category, which is one of our better scores. Our inspection scores are based on several pieces of information found on the a nursing home's recent inspection reports. Deficiencies are a key thing to look for on these reports. Most importantly, you should avoid nursing homes with severe deficiencies linked to endangerment of patients. While this nursing home had some minor dings on its report, it had zero severe deficiencies. Severe deficiencies are found in categories G through L. This means the inspectors did not deem any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple minor deficiencies are not the end of the world.
Unfortunately, this facility only received a grade of D for its nursing grade, which was actually one of its better category grades. Our nursing rating is largely based on the facility's nurse staffing. This nursing home provides 0.3 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. Finally, we also assessed some nursing quality measures in calculating our nursing ratings. This place performed well when it comes to minimizing its residents' major falls and pressure ulcers. We consider these metrics to be measures of the quality of nursing care.

Long-term Care Quality
Moving on to the area of long-term care, this nursing home received a grade of just D in that area. If you are seeking anything other than short-term rehabilitation, you should take a close look at each nursing home's long-term care grades. In addition to assessing the volume of care provided by nurses and other staff, we also looked at the percentage of residents vaccinated for pneumonia. This nursing home provided the vaccine to 100 percent of its patients. Surprisingly, this nursing home was able to limit hospitalizations. With just 0.03 hospitalizations per 1,000 long-term resident days, this facility had less hospitalizations than many nursing homes. Unfortunately, some of its other scores in the category were not as favorable as these.
Short-term Care Quality
The final area we assessed was short-term care. This nursing home was given an abysmal grade of F in this category. Short-term care ratings are based on the facility's quantity of highly-skilled highly skilled professionals. This includes a wide spectrum of nursing services, ranging from registered nurses to physical and respiratory therapists, in addition to other types of therapy. With its grade in this category, we weren't surprised to learn that this place was well below average in terms of the quantity of registered nurse and physical therapy hours offered to its patients based on the figures we looked at. Honestly, this was what we expected to find. The last measure we looked at in this category is the percentage of patients who were able to leave the facility and return to the community. We discovered that just 2.3 percent of this facility's patients were able to return home, which is well below average. The combination of these poor metrics sunk this nursing home's short-term care score.
