Long-term Care Quality
Although we gave this facility a terrible overall grade, we awarded it a grade of B- for our long-term care rating. Our long-term care scores are more critical for patients requiring more traditional assistance related to personal care. On top of looking at the volume of care provided by aids and other staff, we looked at the percentage of residents vaccinated against pneumonia. This facility provided the vaccine to 100 percent of its residents, which is higher than the average nursing home. Lastly, this facility was also able to limit hospitalizations. Indeed, it had only 0.02 hospitalizations per 1,000 long-term resident days, which is a remarkably low figure.
Short-term Care Quality
We were also pleasantly surprised to discover that this nursing home earned a somewhat respectable short-term care score in our assessment. We gave it a grade of C in this category. Short-term care grades are often used to measure a nursing home's rehabilitation services. In order to have good rehabilitation services, nursing homes generally must feature better levels of highly skilled nursing services. These types of services include registered nurses, physical therapists, speech therapists and other highly trained professionals. In this facility's case, we were surprised to learn that it actually provides less physical therapist and registered nurse hours per resident than the typical facility. Finally, we looked at the percentage of residents who returned home from this nursing home. We found that 0 percent of this facility's patients were able to return home. This figure was quite a bit below average.

Nurse Quality
We gave this facility a grade of just D for our nursing rating. This place averaged only 0.3 hours of nursing care per patient each day. This is an alarmingly low total. Finally, we also looked at some nursing quality-based metrics in calculating our nursing grades. This place fared well in terms of avoiding pressure ulcers and major falls. Many consider these metrics to be measures of the quality of nursing care.
Facility Inspections
This facility did not receive a favorable inspection grade. We gave it a just D in this category. Inspection scores relate to a facility's inspections. We weight poor inspection scores heavily in our assessment. Since the inspection reports are completed by independent government inspectors, we consider this information to be reliable. Our inspection grades weigh a host of factors included in a facility's inspection reports. One key criteria we rely on is the number and severity of deficiencies. This nursing home was assessed 5 deficiencies on its inspection report. The only positive thing we can say is that none of the deficiencies were in the categories that indicate that they caused a threat to resident safety or health. Lastly, this nursing home was cited by CMS for possible abuse or neglect. This is generally a really bad sign. We would never recommend this facility.
