Facility Inspections

Grade: A-plus

This nursing home received a solid overall grade as described above, but we really wanted to emphasize its nearly flawless inspections in recent years. We awarded them an impressive grade of A+ in this category. Inspection grades are based on many items located in the a nursing home's recent inspection reports. Places that receive favorable grades in this category have few deficiencies on their reports. Most importantly, these nursing homes generally do not have any severe deficiencies which are associated with endangerment of patients. Fortunately, although this facility had a few minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are labeled as categories G, H, I, J, K and L. This means that CMS did not deem any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. Finally, this facility had no substantiated complaints this year from residents, which is always a good sign.

Long-term Care Quality

Grade: B-plus

Additionally, we also wanted to note that this nursing home was given positive long-term care marks this year. We gave them one of our better grades in this area, with a B+. This turned out to be the facility's second highest grade. Nursing homes that receive this kind of grade in this category typically provide consistent 24/7 care to ensure patients are kept in good health. After considering the volume of care provided by nurses and other staff, we next considered the facility's vaccination statistics. This nursing home vaccinated 100 percent of its residents for pneumonia, which is more than the vast majority of nursing homes. Finally, this facility was able to limit hospitalizations. Indeed, it had only 0.02 hospitalizations per 1,000 long-term resident days, which is an impressively low figure.

Short-term Care Quality

Grade: F

Another concern was the F this nursing home received in the area of short-term care. In determining our short-term care scores, we assess the facility's levels of highly skilled nursing services, which includes registered nurses, physical therapists speech therapists and other highly trained professionals. This rating is more often than not a fair assessment of the facility's rehabilitation. As you might expect, we learned that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. Finally, we looked at the number of patients who were able to return to the community from this nursing home. This definitely was not a strength for this facility. We found that just 4.2 percent of this nursing home's patients returned home. At most nursing homes, about half of their residents are able to return home so this is well below average.

Nurse Quality

Grade: F

This facility's weakest area is nursing, which is the final area we scored. It received an F for this category. Even with several acceptable grades in other areas, this score is still alarming so we wanted to point it out. Our nursing score is based on the facility's nurse staffing levels. We consider both the levels of training of the nurses and the number of hours spent with patients. This nursing home averaged only 0.3 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. To go along with its low totals in the area of nursing hours per patient, this nursing home also didn't perform as well in the quality-based measures we looked at in computing our nursing scores. We looked at the percent of residents experiencing major falls and pressure ulcers. This facility had more pressure ulcers and falls than the average facility.
