Even though we graded this facility incredibly poorly overall, it received a decent short-term care grade this year. We gave it a B- in that area, which was the closest thing to a bright spot we could find for this nursing home. Our short-term care ratings are considered to be most important for individuals requiring rehabilitation services. Rehabilitation generally utilizes additional highly-skilled nursing. Skilled nursing includes not merely nursing services, but also physical and occupational therapy, as well as other forms of therapy. Unfortunately, we found that this facility provided fewer physical therapist and registered nurse hours per resident than most other facilities. The final measure we assessed in this category is the number of patients who were able to leave the facility and return home. We discovered that just 5.9 percent of this facility's patients were able to return home, which is actually below average.
Nurse Quality
One of the other reasons we rated this facility so highly is that it earned a very impressive nursing score. In fact, nursing turned out to be its second best category grade. In that area, we gave this facility an D. Nursing scores are mostly associated with a nursing home's nurse staffing. This nursing home provided just 0.4 hours of nursing care per patient each day. Lastly, we also looked at some nursing quality-based metrics in calculating our nursing scores. This place performed well in terms of minimizing its residents' major falls and pressure ulcers.

Facility Inspections
Unfortunately, with a grade of D in the inspection category, this facility performed relatively poorly in that area. Our inspection scores factor in many items on the facility's inspection reports. Finally, CMS cited this nursing home for possible abuse or neglect. Hopefully, its performance improves in the future, but until then we simply can't recommend this facility.
Long-term Care Quality
Turning to our last category, this facility really didn't fare well in this category either. Due to a very poor grade of F in long-term care, this is just about as terrible as it gets. If you are seeking anything other than rehabilitation, you should take a close look at each nursing home's long-term care grades. Once we assessed the amount of care provided by nurses aids and other staff, we next considered the nursing home's vaccination records. We were pleased to learn that this nursing home vaccinated 98.75 percent of its residents against pneumonia. To our surprise, this facility was able to keep its patients out of the hospital. Indeed, it had just 0.02 hospitalizations per 1,000 long-term resident days. Sadly, some of its other scores in this category were not as favorable as these.
