Facility Inspections
While we didn't give this place a good overall grade, we actually gave it a relatively strong grade in the area of inspections, where it received a B+. Arguably the most critical factor we consider in determining our inspection ratings is deficiencies. These deficiencies can be found on a facility's inspection reports. It is typically in your best interest to avoid places that have too many deficiencies. This place was assessed 4 deficiencies on its inspection report, but none of the deficiencies were considered to be major deficiencies. This tells you that the inspectors did not deem any of the deficiencies to cause an imminent threat to patient safety or health. A few minor deficiencies aren't the end of the world.
Nurse Quality
We also ended up giving this facility a fairly respectable grade in the area of nursing, where it received a grade of B-. We scrutinized the qualifications of nurses at the nursing home, as well as the quantity of hours the nurses spent with residents, in calculating our grade in this area. This place provides only 0.5 hours of nursing care per resident per day. This is a somewhat alarming figure which is well below average. Lastly, nursing ratings also factor in quality-based measures, such as preventing major falls. Despite not having the most impressive total nursing hours per resident, this place performed admirably in the metric of preventing major falls. This is usually an indicator that a place has sufficient quality controls in place. Many falls can be avoided if a nursing home supplies enough nurses aids to assist its patients.

Long-term Care Quality
We gave this nursing home a grade of just D for our long-term care score. This is not a score to write home about. Facilities that receive this type of grade in this category may not provide the kind of consistent 24/7 care that some other nursing homes offer. On top of considering the volume of care provided by aids and other staff, we also looked at the number of residents vaccinated against pneumonia. This nursing home vaccinated 76.95652 percent of its patients. This is multiple points below what we were hoping for. To our surprise, this facility was actually able to limit hospitalizations. With just 0.01 hospitalizations per one thousand long-term resident days, this facility had fewer hospitalizations than most nursing homes. This is its best score in this category.
Short-term Care Quality
The last category we rated was short-term care. It received an abysmal F in this category, which is a poor score. In the category of short-term care, we try to qualify indicators of a facility's rehabilitation. We look at the nursing home's skilled nursing services, such as those performed by registered nurses and physical and occupational therapists. Based on its abysmal score in this area, we weren't surprised to discover that this place provides significantly fewer physical therapist and registered nurse hours per resident than most other facilities. The final item we looked at in this category is the percentage of residents that ultimately returned home from the facility. This nursing home struggled quite a bit in this metric as well, with just 3.8 percent of its patients returning home.
