Facility Inspections

Grade: A-plus

The main reason this nursing home received a decent overall grade is its impeccable government inspections in recent years. We awarded it one of our better grades in that category, with an A+. This is a notably better grade than the facility's overall score, which was decent but certainly not elite. Perhaps the most important factor we consider in calculating our inspection grades is deficiencies. These deficiencies are found on a facility's recent inspection reports. Places with higher grades in this area typically dodged the more severe deficiencies involving patient abuse or death. Amazingly, this was one of the few nursing homes in the country that had no deficiencies whatsoever on its inspection report. This is very impressive. This is certainly a good sign for the quality of care at this nursing home.

Long-term Care Quality

Grade: B-plus

Additionally, we also wanted to point out that this facility received favorable long-term care marks this year. We gave them one of our better grades in this area, with a B+. This proved to be the nursing home's second highest grade. When nursing homes receive a grade in this range in this category it typically means it has plenty of staff and is an overall good place to reside on a permanent basis. In addition to assessing the volume of care provided by nurses and other staff, we analyzed the percentage of residents vaccinated for pneumonia. This facility provided the vaccine to 100 percent of its residents. Vaccines are vital to keeping patients healthy. Finally, this nursing home was able to limit hospitalizations. It had less than one hospitalization per one thousand long-term resident days, which is an impressively low figure.

Nurse Quality

Grade: F

Next we looked at nursing grades, where this nursing home received an F, which is a rock bottom grade. This area was definitely not a strength for this facility. When computing our nursing scores, we weigh both nursing hours and the training levels of those nurses. This facility averaged a meager 0.3 hours of nursing care per patient per day. Finally, we also assessed some nursing quality-based metrics in computing our nursing scores. This nursing home fared well in the area of avoiding pressure ulcers and major falls. We consider these statistics to be good indicators of the quality of nursing care.

Short-term Care Quality

Grade: F

The last area we analyzed was short-term care. It received an abysmal F for this area, which is a bottom of the barrel grade. This is certainly a significant disappointment. In the category of short-term care, we try to qualify indicators of a facility's rehabilitation. We assess the facility's skilled nursing services, such as the ones performed by registered nurses and physical therapists. Considering its poor grade in this category, we were not stunned to discover this facility provided significantly fewer physical therapist and registered nurse hours per resident than most other nursing homes. Lastly, we looked at the percentage of patients who were able to return to the community from this facility. We found that just 2.8 percent of this nursing home's residents returned home.

Audubon Health and Rehab Quality Metrics

Minimizes Pressure Ulcers

Grade: D

In Audubon Health and Rehab, 13.95% of Patients had Pressure Ulcers

This figure is a measure of the percentage of long-term care patients who have new or worsened pressure ulcers or bed sores. We consider this statistic when determining our nursing grades.

Minimizes Serious Falls

Grade: D

In Audubon Health and Rehab, 6% of Patients had Serious Falls

This is a measure of the percent of long-term stay residents who suffered falls resulting in severe injury. This is one of the statistics we use in determining our nursing ratings.

Minimizes Urinary Tract Infections

Grade: B

In Audubon Health and Rehab, 1.52% of Patients had UTIs

This indicates the percent of residents that have had a urinary tract infection. UTI's are routinely the result of facilities with lower levels of hygiene. Closer supervision can reduce the percentage of residents in a nursing home who suffer from UTI's. Keep in mind that this metric is sometimes skewed by the fact that facilities have incongruent reporting standards for UTI's.

Appropriately Uses Anti-Psychotic Medication

Grade: C

In Audubon Health and Rehab, 21.78% of Patients use Anti-Psychotic Medication

This is the percent of patients given antipsychotic medications. While antipsychotic drugs may be vital to many patients, it is important to make sure these drugs are being used appropriately. In limited situations, increased usage of these drugs may suggest that a nursing home is using these drugs to control patient behavior.

Appropriately Uses Anti-Anxiety Medication

Grade: B-minus

In Audubon Health and Rehab, 23.15% of Patients use Anti-Anxiety Medication

This statistic is a measure of the percent of long-term care patients who were prescribed antianxiety drugs. These drugs are prescribed to residents suffering from depression and anxiety.

Managing Depression Among Residents

Grade: B

In Audubon Health and Rehab, 2.98% of Patients

This indicates the percentage of patients exhibiting symptoms of depression.

Appropriate Vaccine Usage

Grade: A

In Audubon Health and Rehab, 100% of Patients

This tells you the percentage of patients that received the flu and pneumonia vaccines. Higher vaccination rates should be the norm at this point.

Residents Maintain Autonomy

Grade: C

In Audubon Health and Rehab, 19.71% of Patients

This tells you the percent of residents who needed more assistance with activities of daily living over time. Higher levels of dependence on staff for assistance with activities of daily living could be a sign of decline of a resident's well-being.

Ability to Keep Residents Mobile

Grade: B-minus

In Audubon Health and Rehab, 13.46% of Residents

This indicates the percentage of residents who retained mobility levels over time. Optimizing mobility is often a good sign for residents' health.

Hospitalizations

Grade: C

In Audubon Health and Rehab, 0.02 Hospitalizations per 1,000 resident days

This is the number of hospitalizations per thousand days of patient care. There is typically a correlation between keeping patients out of the hospital and the quality of nursing home care.

Short-term Care: Rehospitalizations

Grade: B-plus

In Audubon Health and Rehab, 13.46% of Residents Rehospitalized

This metric measures the number of times residents are rehospitalized per thousand days of short-term care.

Short-term Care: ER Visits

Grade: D

In Audubon Health and Rehab, 17.33% of Patients

This datapoint measures the number of times residents are sent to the emergency room per thousand days of short-term patient care.
