Facility Inspections

Grade: B-plus

While this place's overall score was as bad as it gets, it fared pretty well in the category of inspections. In fact, we awarded it a grade of B+ for that category, which is one of our better scores. Inspection scores are based on many items located in the a nursing home's recent inspection reports. Deficiencies are an important thing to look for on these reports. You should especially avoid nursing homes that have severe deficiencies associated with endangerment of patients. Although this facility had some deficiencies on its report, none of them were major deficiencies based on CMS' scale. A few minor deficiencies are not necessarily the end of the world.

Long-term Care Quality

Grade: C

We were also stunned to learn that this facility received a solid long-term care grade in our assessment. We gave it a C in this area. For patients looking for a permanent place to live as opposed to rehabilitation, long-term care is a very important category. One of the criteria we considered on top of nurse's aid hours is vaccinations. This facility administered the pneumonia vaccination to 93.53234 percent of its patients. Nevertheless, this place excelled at keeping its patients out of the hospital. Indeed, it had less than one hospitalization per 1,000 long-term resident days, which is an impressively low figure.

Nurse Quality

Grade: D

Turning to the area of nursing care, this facility received a grade of just D in this category. Our nursing rating assesses the nursing home's nurse staffing levels. We look at both the levels of training of those nurses and the quantity of hours spent with residents. With a meager 0.3 hours of nursing care per patient each day, this place's nurse staffing levels were significantly below the national average. Finally, we also looked at some nursing quality-based metrics in computing our nursing grades. This nursing home performed well when it comes to minimizing its residents' major falls and pressure ulcers. Many consider these datapoints to be good measures of the quality of nursing care.

Short-term Care Quality

Grade: F

The next area we graded was short-term care. Sadly, it received an F in this category, which is a rock bottom grade. Our short-term care grade is often employed to judge a nursing home's performance with rehabilitation. In order to provide highly graded rehabilitation services, nursing homes generally need to feature higher levels of highly skilled nursing. These types of services include registered nurses, physical therapists, occupational therapists and other highly skilled individuals. Based on its abysmal score in this area, we weren't stunned to discover that this place provided substantially fewer physical therapist and registered nurse hours per patient than most other nursing homes. The final measure we looked at in this area is the percentage of patients that ultimately returned home from the nursing home. This nursing home struggled quite a bit in this metric as well, with just 4.9 percent of its patients returning home. Unfortunately, this was quite a bit below the national average.

Minimizes Pressure Ulcers

Grade: A

In Waldron Rehabilitation & Healthcare Center, 4.4% of Patients had Pressure Ulcers

In Waldron Rehabilitation & Healthcare Center, 9.08% of Patients

This is the number of emergency room visits per thousand days of short-term patient care. There is usually a correlation between having fewer emergency room visits and the overall quality of short-term care.

Short-term Care: Facilitates Functional Improvement

Grade: A-plus

In Waldron Rehabilitation & Healthcare Center, 100% of Resident
