Facility Inspections
While this nursing home's overall grade was terrible, it performed well in the area of inspections. In fact, we awarded it a B+ for that category, which is one of our better scores. Arguably the most critical factor we consider in calculating our inspection grades is deficiencies. Deficiencies are found on a facility's inspection reports. It is generally best to avoid places that have too many severe deficiencies. This facility was hit with 8 deficiencies on its inspection report, but we were relieved to see that none were considered severe. This tells you that the government inspectors didn't deem any of these deficiencies to create an immediate threat to resident safety or health. A couple minor deficiencies aren't the end of the world.
Long-term Care Quality
Sadly, this nursing home's next most respectable score was long-term care. However, with a grade of D in this category, it still performed relatively badly. For residents seeking a permanent place to live as opposed to skilled nursing, long-term care is an important category. After assessing the amount of care provided by nurses aids and other staff, we then looked at the nursing home's vaccination records. We were pleased to find that this facility vaccinated 96.385544 percent of its residents for pneumonia. Surprisingly, this place was able to limit hospitalizations. With just 0.01 hospitalizations per one thousand long-term resident days, this nursing home has fewer hospitalizations than the majority of nursing homes. Unfortunately, a few of its other scores in this category weren't as favorable as these.

Short-term Care Quality
The third category we looked at was short-term care. This nursing home's received an unfortunate short-term care grade where it received an F. With our short-term care grade, we seek to craft a fair barometer for rehabilitation services. In this process, we assess the facility's levels of skilled nursing services, including both registered nurses and physical therapy, as well as respiratory therapy. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. The last metric we considered in this category is the number of patients who were able to leave the facility and return to the community. We discovered that just 0 percent of this facility's residents were able to return home, which is well below average. The combination of these poor statistics was damning for this facility's short-term care score.
Nurse Quality
The final area we graded is nursing. Unfortunately, we gave it an F for this area, which is a bottom of the barrel grade. In computing our nursing ratings, we assess both staffing levels and the levels of licensure of the nurses. This place offered a meager 0.4 hours of nursing care per patient per day. This is not a very impressive figure. Lastly, we also looked at some nursing quality-based metrics in determining our nursing ratings. This place performed well when it comes to minimizing its residents' pressure ulcers and major falls. We consider these metrics to be good indicators of the quality of nursing care.
