Facility Inspections
Although this place's overall score was as bad as it gets, it actually performed pretty well in the area of inspections. In fact, we awarded it a grade of B for that category, which is one of our better scores. We weigh several aspects of a nursing home's inspection report in determining our inspection scores. One critical factor is health deficiencies. You should keep in mind that the severity of these deficiencies is usually more important than the number of deficiencies, as some end up being quite minor. Although this nursing home had a few deficiencies on its report, none of them were major deficiencies based on CMS' scale. A couple minor deficiencies are not the end of the world.
Short-term Care Quality
Sadly, this nursing home's second most respectable category is short-term care. Nevertheless, with a grade of D in this area, it still performed somewhat badly. With our short-term care category, we strive to forge a valuable barometer for rehabilitation. In this process, we look at the facility's level of skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. Not surprisingly, we found that this nursing home offers far less registered nurse and physical therapist hours per patient than the typical facility. Finally, we considered the percentage of residents that who were able to eventually return home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 4.9 percent of this facility's residents were able to return home.

Long-term Care Quality
Our third category we looked at was long-term care, in which this facility was given an F. When facilities receive this type of grade in this category it is often a bad sign for resident care and it may indicate that the nursing home is not as well-staffed. After considering the amount of care provided by aids and other staff, we turned to the facility's vaccination record. Frankly, we were a bit alarmed this nursing home vaccinated 87.864075 percent of its patients for pneumonia, which happens to be several points lower than average. Surprisingly, this nursing home actually fared well at keeping its patients out of the hospital. In fact, it had only 0.02 hospitalizations per 1,000 long-term resident days. This is its best score in this category.
Nurse Quality
Turning to our fourth category, this facility really did not perform very well in this area either. With an abysmal grade of F in nursing, this is about as bad as it gets. Nursing grades are based in large part on levels of nurse staffing. With only 0.3 hours of nursing care per resident each day, this place's nurse staffing levels were far below the national average. In addition to receiving below average marks for nursing hours, this facility was less impressive in some of the quality-based metrics we looked at in calculating our nursing ratings. We looked at the percent of residents suffering pressure ulcers and found this facility was at approximately 1.5 times the national average in this statistic.
