Facility Inspections

Grade: A-minus

While we graded this facility incredibly poorly overall, it earned a very impressive government inspection report this year. As a result, it received one of our best grades in that category with an A-. Inspection grades are based on pieces of information located in the a nursing home's recent inspection reports. Deficiencies are a key thing to look for on these inspections. Most importantly, you should avoid nursing homes with severe deficiencies associated with endangerment of patients. Fortunately, although this place had a few minor dings on its report, it had no severe deficiencies. Severe deficiencies are ones labeled as categories G, H, I, J, K and L. This means that CMS did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple minor deficiencies are not necessarily the end of the world.

Short-term Care Quality

Grade: D

Sadly, this facility only earned a D for its short-term care rating, which turns out to be one of its higher category grades. Our short-term care ratings are considered to be more critical for folks requiring rehabilitation from their nursing home. Rehabilitation usually requires more highly-skilled nursing. This includes not just nursing, but also physical and occupational therapy, as well as other types of therapy. Not surprisingly, we found that this nursing home provides far less physical therapist and registered nurse hours per resident than the average facility. Lastly, we assessed the percentage of patients that eventually returned home from this facility. This nursing home didn't fare well here either. In fact, we found that just 4.6 percent of this facility's residents were able to return home. This figure was quite a bit off the national average.

Long-term Care Quality

Grade: D

This facility's second worse category is long-term care, where it received just a D in that category. This is a well below average grade. In a long-term care setting, the nursing home's primary objective is to keep patients as healthy and safe as possible. This is different than skilled nursing or short-term care where the goal is to rehabilitate residents. In addition to considering the volume of care provided by aids and other staff, we also looked at the percentage of residents vaccinated for pneumonia. This facility administered the vaccine to 98.01588 percent of its residents. Surprisingly, this facility also keeps its patients out of the hospital. Indeed, it had only 0.02 hospitalizations per 1,000 long-term resident days. Unfortunately, some of its other scores in the category weren't as favorable as these.

The last category we looked at was nursing, in which this facility was given a rock bottom grade in this area as well. Sadly, it received an F for this category, which is definitely a major concern. Nursing grades are largely tied to a facility's nurse staffing. This facility provided a meager 0.3 hours of nursing care per resident each day. Finally, we also looked at some nursing quality-based metrics in computing our nursing grades. This place performed well in the area of avoiding pressure ulcers and major falls.
