Facility Inspections
On top of performing well in its overall grade, this facility performed at an even higher level in the area of inspections. In fact, it received a nearly flawless inspection report this year. As a result, it received one of our highest grades in that category with a score of A+. Perhaps the most significant factor we look at in computing our inspection scores is deficiencies. Deficiencies are found on a nursing home's recent inspection reports. Facilities with better scores in this category typically dodged the more severe deficiencies involving health risks or even death. This place was assessed 3 deficiencies on its inspection report, but we were relieved to see that none were considered to be severe deficiencies. This tells you that CMS didn't consider any of the deficiencies to be an imminent threat to patient health or safety. A couple relatively minor deficiencies are not necessarily the end of the world.
Long-term Care Quality
This nursing home received a favorable long-term care grade as well. We awarded it one of our higher scores in that area with a grade of B+. Facilities that do well in long-term care tend to be well-staffed and offer extensive hands on care to residents. After assessing the amount of care provided by nurses aids and other staff, we then looked at the nursing home's vaccination records. This nursing home vaccinated 100 percent of its patients for pneumonia, which is significantly higher than the vast majority of nursing homes. Clearly, this place is doing something right in this area as it also excelled at keeping its residents out of the hospital. It had only 0.02 hospitalizations per one thousand long-term resident days, which is an impressively low figure.

Short-term Care Quality
This nursing home was given its next best category score in the category of short-term care. We awarded this facility a B- in this area, which is an acceptable score. Our short-term care scores are vital for patients needing rehabilitation. Rehabilitation generally requires higher levels of skilled nursing. This includes a broad range of services, ranging from registered nurses to physical and occupational therapists, as well as other forms of therapy. Fortunately, it looks like this facility has registered nurses on staff. Not every facility employs these skilled professionals. On the other hand, according to the data this nursing home provided, it does not look like the facility employs physical therapists. Lastly, we considered the number of patients who were able to return to the community from this facility. We found that 4.5 percent of this facility's patients returned home. At most facilities, closer to half of their patients are able to return home so this figure is below average.
Nurse Quality
Our last category is nursing, where we awarded this facility a C in that category. The nursing grade weighs a number of data points. The most important factor is the amount of time nurses spend with patients. This place averages just 0.3 hours of nursing care per patient on a daily basis, which is a very low figure. Finally, we like to look at quality-based metrics, such as the percentage of patients who suffered pressure ulcers and major falls. These statistics are measures of the quality of nursing care.
