Facility Inspections

Grade: A

This facility earned a solid overall grade as described above, but we really wanted to emphasize its excellent health inspection reports in recent years. We gave them an impressive grade of A in this category. Arguably the most critical factor we consider in determining our inspection scores is deficiencies. Deficiencies can be found on a nursing home's recent inspection reports. Nursing homes with better scores in this category typically dodged the most severe deficiencies involving health risks or even death. This nursing home was assessed 7 deficiencies on its inspection report, but none of the deficiencies were considered to be severe. This indicates that CMS didn't deem any of the deficiencies to create an immediate risk to patient health or safety. A couple of minor deficiencies shouldn't stop you from considering a nursing home.

Long-term Care Quality

Grade: B-minus

This nursing home was awarded its second best category grade in the category of long-term care. We gave this facility a grade of B- in this category, which is a respectable score. Our long-term care grade is used to gauge a facility's performance as a traditional nursing home, as opposed to focusing on the skilled nursing services offered by a rehabilitation facility. In addition to assessing the quantity of care provided by nurses and other staff, we looked at the percentage of residents vaccinated for pneumonia. Frankly, we were a bit alarmed this facility gave the vaccine to a relatively low 86.51163 percent of its residents. Nevertheless, this facility keeps its residents out of the hospital. It had only 0 hospitalizations per one thousand long-term resident days, which is a remarkably low figure.

Short-term Care Quality

Grade: D

We gave this nursing home a grade of just D for our short-term care grade. This is not a very good score. In determining our short-term care grades, we analyze the facility's levels of skilled nursing services, which includes registered nurses, physical therapists speech therapists and other highly trained professionals. This grade is generally a useful measure of a facility's rehabilitation services. With a grade of this caliber for this area, we weren't shocked to learn that this facility was well below average in multiple staffing metrics we assess. In fact, it supplied far fewer physical therapist and registered nurse hours per patient than most other nursing homes. Finally, we looked at the percentage of residents who were able to return to the community from this nursing home. We found that just 4.8 percent of this nursing home's patients returned home.

Nurse Quality

Grade: F

The final category we rated is nursing. It received an abysmal F for this area, which is an abysmal score. This is certainly a significant concern. Nursing grades are based largely on quantity of nursing care available. With a meager 0.3 hours of nursing care per patient each day, this facility's nurse staffing levels were far below average. To pair with its below average totals in the area of nursing hours per resident, this place had really bad scores in many of the quality-based measures we looked at in calculating our nursing scores. For starters, we looked at the percent of patients sustaining falls leading to serious injury. This place was at more than 150 percent of the national average in this statistic. This is concerning in light of the fact that many falls could be prevented with better nursing care. This ended up sealing the facility's fate of earning a poor nursing grade.
