Facility Inspections

Grade: F

This place's grades were horrible across the board. We especially want to draw your attention to its inspection grade where it received an F. This isn't a good sign for its government inspection reports. For nursing homes that received this poor of an inspection score, we would scrutinize any severe deficiencies on its inspection report. This nursing home had a category G through L deficiency, which rank among the more serious categories of deficiencies. These categories indicate that the deficiencies identified by CMS had potential to pose actual harm to patients. Lastly, we would like to draw your attention to the fact that this nursing home was assessed large government fines in recent years amounting to more than $100,000. This generally means that a facility's poor track record spanned several years.

Nurse Quality

Grade: F

We also wanted to point out that this nursing home received an F for its nursing grade. We looked at the licensure of nurses working for the facility, as well as the number of hours those nurses worked with patients, in calculating our grade in this category. This place provided only 0.3 hours of nursing care per resident each day. This is not a very impressive figure. Lastly, we also assessed some nursing quality measures in determining our nursing grades. This place performed well in terms of avoiding pressure ulcers and major falls. We consider these statistics to be indicators of the quality of nursing care.

Short-term Care Quality

Grade: F

Another concern is the F this nursing home received in the category of short-term care. With our short-term care rating, we attempt to craft a sound measure for rehabilitation. In doing so, we analyze a facility's level of highly-skilled nursing services, including both registered nurses and physical therapy, as well as respiratory therapy. With its score in this category, we weren't stunned to find that this place was well below average in terms of its quantity of physical therapy and registered nurse hours offered to its residents based on the metrics we assessed. Frankly, this was what we expected to find. Lastly, we assessed the percentage of residents that ultimately returned home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 4.8 percent of this facility's residents were able to return home.

Long-term Care Quality

Grade: F

The last category we rated was long-term care, in which this facility was given a rock bottom score in this category also. We gave it an abysmal F in this category, which is obviously very concerning. When facilities receive a grade in this range in long-term care it typically means the nursing home did not perform well in our quality measures relating to resident care. On top of looking at the quantity of care provided by nurses aids and other staff, we also looked at the percentage of residents vaccinated for pneumonia. This facility vaccinated 85.645935 percent of its residents. This is pretty significantly less than average. To our surprise, this facility actually fared well at keeping its patients out of the hospital. Indeed, it had just 0.02 hospitalizations per one thousand long-term resident days. This was its best feature in this category.
