Long-term Care Quality

Grade: A

One of the many reasons this turned out to be a great nursing home is that it received a very impressive long-term care rating. This ended up being its best category. In this category, we gave this facility an A. When nursing homes receive a grade in this range in this category it typically means it's well-staffed and is an overall good place to live on a permanent basis. One of the datapoints we considered after nursing hours was vaccines. This nursing home provided the pneumonia vaccination to 100 percent of its patients. Vaccination is a great way to avoid negative health outcomes for the nursing home population. Finally, this facility was able to limit hospitalizations. Indeed, it had less than one hospitalization per one thousand long-term resident days, which is a remarkably low number.

One of the other reasons we graded this facility so highly is that it received a very impressive nursing grade. This proved to be its second best category grade. In that category, we gave this nursing home a grade of A. There are quite a few datapoints within this category. Many of the factors relate to nurse staffing. This facility provided 0.3 hours of nursing care per resident per day. Finally, we also assessed some nursing quality measures in determining our nursing grades. This facility performed very well in terms of avoiding major falls and pressure ulcers. We consider these areas to be good indicators of the quality of nursing care.

Short-term Care Quality

Grade: A

The next area we analyzed is short-term care. Contributing to its superb profile, this nursing home also performed well in this area. In fact, we gave it a of A in the area. In crafting these short-term care grades, we analyze the nursing home's skilled nursing services, including those performed by registered nurses, speech therapists, physical therapists and other types of therapists. Our goal is to create a measure for comparing the rehabilitation services of various nursing homes. Fortunately, it appear that this nursing home has registered nurses on staff. Not all facilities employs these types of nurses. On the other hand, based on the data they provided, they do not appear to employ physical therapists. Lastly, we considered the number of residents who were able to return to the community from this nursing home. We found that 0 percent of this facility's patients returned home.

Facility Inspections

Grade: A-minus

Lastly, this nursing home also earned a virtually flawless inspection. As a result, it received one of our highest grades in that category with a grade of A-. With this impressive grade, this place is one of the few nursing homes in Minnesota to earn straight A's. Our inspection scores weigh several factors included in a nursing home's inspection report. One of the most important criteria we rely on is the quantity and severity of deficiencies. Facilities with better scores in this category tend to have very few of these severe deficiencies. While this place had a few minor dings on its government inspection report, it had zero severe deficiencies. Severe deficiencies are ones found in categories G, H, I, J, K and L. This tells you that CMS didn't consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.
