Short-term Care Quality

Grade: A

In addition to being a quality facility overall, this facility excelled in our short-term care rating, where it received an A. In determining our short-term care grades, we scrutinize a nursing home's levels of skilled nursing, which includes registered nurses, physical therapists respiratory therapists and other highly skilled individuals. This score is generally a reliable measure of the nursing home's rehabilitation. Fortunately, it appear that this nursing home has registered nurses on staff. Not all facilities employs these types of nurses. However, based on the data this nursing home provided, it does not look like the facility employs physical therapists. The last metric we assessed in this category is the number of residents that returned home from the facility. We found that 5.5 percent of this nursing home's patients returned home as opposed to remaining at the facility on a permanent basis.

Nurse Quality

Grade: A-minus

One of the other reasons we rated this facility so highly is that it received a very impressive nursing grade. In fact, nursing turned out to be its second best category grade. In that area, we awarded this facility a grade of A-. Nursing grades are mostly tied to the nursing home's level of nurse staffing. This nursing home provides 0.3 hours of nursing care per resident per day. Finally, we also looked at several nursing quality-based metrics and this facility excelled in some of these areas. With less than five percent of its residents sustaining pressure ulcers, it performed as well as any nursing home Minnesota in this category. This is generally a good indicator of quality nursing care. Pressure ulcers can often be prevented by providing better nursing care, such as by employing a policy of turning residents at least once a day.

Facility Inspections

Grade: B

Adding to its strong resume, this facility earned a grade of B for its inspections rating. This is a well above average score in this category. Perhaps the most critical factor we look at in determining our inspection ratings is deficiencies. These deficiencies can be found on a facility's inspection reports. Places with better scores in this area most likely dodged the more severe deficiencies involving things like patient abuse. While this place had some minor dings on its inspection report, it had no severe deficiencies. Severe deficiencies are found in categories G, H, I, J, K and L. This means CMS didn't consider any of the deficiencies on this facility's report to pose an imminent threat to patient health or safety. A few relatively minor dings are not necessarily the end of the world.

Long-term Care Quality

Grade: C

The final category we scored was long-term care. In that category, we awarded this facility a grade of C. In a long-term care setting, the primary goal is to keep patients as healthy and safe as possible. This is different than skilled nursing or short-term care where the goal is to rehabilitate residents. After assessing the amount of care provided by nurses aids and other staff, we turned to the nursing home's vaccination statistics. We were optimistic to find that this facility vaccinated 100 percent of its residents against pneumonia. This nursing home keeps its residents out of the hospital. Indeed, it had less than one hospitalization per one thousand long-term resident days, which is a remarkably low number.
