Although we graded this facility extremely poorly overall, it received a better long-term care grade this year. We gave it a C in that category, which was the closest thing to a bright spot we could find for this facility. If you are looking for anything other than short-term rehabilitation, you should take a close look at each nursing home's long-term care ratings. One of the datapoints we considered on top of nursing hours was vaccinations. Thankfully, this facility provided the pneumonia vaccination to 100 percent of its patients. This facility also excelled at keeping its patients out of the hospital. It had only 0.07 hospitalizations per one thousand long-term resident days, which is an impressively low number.

Unfortunately, this facility only received a grade of D for its nursing grade, which was actually one of its better category grades. We scrutinized the skill-level of nurses at the nursing home, in addition to the number of hours those nurses were with patients, in computing our score in this category. This nursing home provides 0.6 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. Finally, we also assessed some nursing quality measures in determining our nursing ratings. This nursing home fared well when it comes to avoiding major falls and pressure ulcers. We consider these metrics to be good measures of the quality of nursing care.

Unfortunately, with a grade of D in the inspection category, this facility performed relatively poorly in that area. Inspection ratings factor in many items on the facility's inspection reports. When you find severe deficiencies on the reports, this is generally the worst sign. Severe deficiencies indicate a risk to resident safety. This particular nursing home was assessed one category G through L deficiency, which are among the more severe deficiencies. This generally indicates that the deficiencies uncovered by the government inspectors had potential to create actual harm to residents. Lastly, we also would like to point out that this nursing home received significant government fines totaling more than $100,000. This usually means that a bad performance has been an issue for several years.

The last area we graded was short-term care. This nursing home was given a rock bottom grade of F in this area. Our short-term care ratings are based in part on the facility's quantity of skilled highly skilled professionals. This includes a wide range of nursing services, ranging from registered nurses to physical and occupational therapists, not to mention other forms of therapy. Considering its weak grade in this area, we were not shocked to learn that this nursing home offered far fewer registered nurse and physical therapist hours per resident than most other facilities. Lastly, we assessed the percentage of residents who who were able to eventually return home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 2.8 percent of this facility's residents were able to return home.
