While this facility's overall grade was as bad as it gets, it actually excelled in the area of inspections. In fact, we gave it a grade of A- for that category, which is one of our highest scores. Inspection ratings account for a host of factors found on a facility's inspection report. One of the most important criteria we look at is the number and severity of deficiencies. You generally want to avoid facilities with a list of deficiencies flagged. While this facility had some minor dings on its inspection report, it had zero severe deficiencies. Severe deficiencies are ones labeled as categories G through L. This means CMS did not consider any of the deficiencies on this facility's report to pose an imminent threat to patient safety or health. A couple minor deficiencies are not the end of the world.

Unfortunately, we gave this facility an F for its nursing grade. Our nursing score is primarily associated with the facility's nurse staffing. This nursing home provides 2.8 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. Lastly, we also looked at some quality measures in determining our nursing grades. This nursing home was above average in two of the areas we considered, with good scores for avoiding pressure ulcers and major falls. These datapoints are often reliable indicators of the quality of nursing care available.

In addition, we gave this nursing home an F in the area of long-term care. For long-term care residents, the nursing home's primary objective is to maintain patients' quality of life and keep them safe. On top of assessing the amount of care provided by nurses and other staff, we also looked at the percentage of residents vaccinated for pneumonia. This nursing home gave the vaccine to 98.63014 percent of its patients. Surprisingly, this nursing home was able to limit hospitalizations. With only 0.98 hospitalizations per one thousand long-term resident days, this facility has fewer hospitalizations than the average nursing home. Sadly, a few of its other scores in the category weren't as favorable as these.

The final category we graded is short-term care. We gave it an F in this area, which is a rock bottom grade. In calculating these short-term care scores, we analyze the nursing home's skilled nursing services, such as those performed by registered nurses, occupational therapists, physical therapists and other types of skilled professionals. Our purpose is to create a barometer for comparing the rehabilitation services of different facilities. Given its score in this area, we weren't stunned to learn that this facility is well below average in terms of its quantity of registered nurse and physical therapy hours provided to its residents based on the figures we looked at. Lastly, we looked at the number of residents who ultimately were able to return home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 31.4 percent of this facility's residents were able to return home. This figure was quite a bit off the national average.
