Facility Inspections

Grade: A

On top of performing well in its overall grade, this facility performed at an even higher level in the area of inspections. In fact, it received a nearly flawless inspection report this year. As a result, it received one of our highest grades in that category with a score of A. We take several aspects of a facility's inspection report into consideration in calculating our inspection ratings. One of those factors is deficiencies. It should be noted that the severity of deficiencies is usually more meaningful than the number of deficiencies, as some of these can be quite insignificant. This particular nursing home had just a single deficiency on its inspection report and it was not one deemed to pose a threat to patient health or safety. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

Long-term Care Quality

Grade: A

This facility performed well in the category of long-term care. In fact, we gave it a grade of A for that category, which is one of our highest scores. Nursing homes that excel in long-term care typically are well-staffed and offer extensive hands on care to patients. On top of assessing the quantity of care provided by aids and other staff, we also looked at the number of residents vaccinated for pneumonia. This nursing home administered the vaccine to 100 percent of its residents, which is much higher than the majority of nursing homes. Lastly, this facility was able to limit hospitalizations. It had less than one hospitalization per 1,000 long-term resident days, which is a remarkably low number.

Nurse Quality

Grade: B

This facility received a very good nursing grade this year as well. It received one of our better scores in that category with a grade of B. Our nursing score looks at the facility's nurse staffing levels. We factor in both the levels of training of those nurses and the number of hours spent with residents. This particular nursing home provided 0.3 hours of nursing care per resident per day. Finally, we also assessed several nursing quality measures and this facility excelled in some of these areas. With less than five percent of its residents sustaining pressure ulcers, it performed as well as any facility the state in this category. This is generally a good indicator of quality nursing care. Pressure ulcers can many times be avoided by offering better nursing care, such as employing a protocol of moving residents at least once a day.

Short-term Care Quality

Grade: B

The last category we rated was short-term care. In that area, we awarded this nursing home a strong grade of B. In our short-term care grade, we endeavor to forge a fair gauge for rehabilitation. In doing so, we assess the facility's scope of highly-skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. Fortunately, it looks like this facility has registered nurses on staff. Not all facilities employs these types of nurses. However, based on the information this nursing home provided, it does not look like the facility employs physical therapists. Finally, we considered the percentage of residents who were able to return to the community from this facility. We found that 4.9 percent of this facility's residents returned home.
