Located in Superior, Wisconsin, Twin Ports Health Services is one of only three nursing homes in this city. This is a grade A facility, which is a phenomenal grade. We were so impressed with this facility that we ranked it in the top 20 percent of all nursing homes in the country. Facility Inspections

Grade: A-plus

We also wanted to point out that this nursing home has received near flawless health inspections in recent years. We awarded them an A+ in this category. Inspection ratings weigh several factors, including deficiencies and federal fines. You can learn more about each of these items by obtaining copies of nursing homes' inspection reports. It does not look like we have deficiency counts for this nursing home. Finally, this facility had no substantiated complaints this year from residents, which is an excellent sign.

Long-term Care Quality

Grade: A

Adding to its dominant performance in other areas, we gave this nursing home a score of A for its long-term care grade. Facilities that receive this type of grade in long-term care tend to provide consistent 24/7 care to make sure residents are well cared for. In addition to considering the volume of care provided by nurses aids and other staff, we also looked at the number of residents vaccinated for pneumonia. This facility provided the vaccine to 100 percent of its residents, which is what we like to see. Pneumonia sadly can be a deadly condition for nursing home patients so we prefer when a nursing home doesn't leave its residents vulnerable. Clearly, this place is doing something right in this area as it was able to keep its patients out of the hospital. It had less than one hospitalization per one thousand long-term resident days, which is an impressively low number.

Nurse Quality

Grade: A-minus

We also wanted to emphasize the fact that this facility received an impressive grade of A- in our nursing category. Nursing ratings are tied to quantity of nursing care available. This particular nursing home provided 0.3 hours of nursing care per resident per day. Finally, our nursing grades also factor in quality-based assessments, such as avoiding major falls. This facility performed well in this area. Avoiding major falls is typically a good indicator that a nursing home has reliable quality controls in place. Major falls can often be prevented if more nurses aids and better safety protocols are in place.

Short-term Care Quality

Grade: B-minus

The last category we assessed ended up being this nursing home's weakest area. However, even in its weakest link we still gave it a decent grade of B- in our short-term care category. In our short-term care category, we seek to craft a meaningful barometer for rehabilitation. In this process, we assess a nursing home's level of skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. Fortunately, it appear that this nursing home has registered nurses on staff. Not all facilities employs these types of nurses. On the other hand, based on the information they provided, they do not appear to employ physical therapists. The final item we considered in this category is the percentage of patients who returned home from the facility. We found that just 3.8 percent of this nursing home's patients returned home. This figure was below average.
