Facility Inspections

Grade: B-plus

Although this facility's overall grade was as bad as it gets, it actually fared pretty well in the area of inspections. In fact, we awarded it a B+ for that area, which is one of our better scores. We weigh several aspects of a nursing home's inspection report in computing our inspection grades. One of those factors is health deficiencies. You should keep in mind that the severity of these deficiencies is usually more meaningful than the number of deficiencies, as some can be quite insignificant. This nursing home was hit with 4 deficiencies on its inspection report, but fortunately none were considered to be severe. This tells you that the inspectors did not consider any of the deficiencies an imminent threat to patient safety or health. A few minor deficiencies are not necessarily the end of the world.

Long-term Care Quality

Grade: D

This facility's second highest score was long-term care, but it still earned just a D in that category. This ended up being a relatively poor grade. Nursing homes that do not fare well in long-term care often do not provide as much nursing care and also may be lagging in a few of the areas of routine medical care we looked at. On top of considering the volume of care provided by aids and other staff, we analyzed the number of residents vaccinated against pneumonia. Candidly, we were a bit concerned this facility gave the vaccine to only 39.577835 percent of its residents. Surprisingly, this facility was actually able to limit hospitalizations. With just 0 hospitalizations per 1,000 long-term resident days, this nursing home has less hospitalizations than the average nursing home. This is its best score in this category.

Short-term Care Quality

Grade: F

Tragically, we gave this facility an F for its short-term care score. Our short-term care scores are crucial for prospective residents in need of rehabilitation. Rehabilitation usually requires additional highly-skilled nursing. This includes a wide range of nursing services, ranging from registered nurses to physical and occupational therapists, as well as other types of therapy. With its grade in this area, we were not stunned to find that this facility was well below average in terms of its number of physical therapy and registered nurse hours offered to its patients based on the data we looked at. Lastly, we assessed the number of patients who were able to return to the community from this nursing home. We found that just 0 percent of this nursing home's residents returned home.

Nurse Quality

Grade: F

The next category we rated is nursing, where this nursing home was given a very poor grade in this area also. It received an abysmal F for this area, which is obviously a major concern. There are quite a few factors included in this area. Most of the criteria reflect the quantity of nurse staffing. This nursing home provides 0.3 hours of nursing care per resident per day. This is a much lower figure than we are used to seeing. In addition to receiving below average marks for total nursing hours, this place was less impressive in some of the quality-based measures we looked at in determining our nursing ratings. We looked at the percentage of residents sustaining pressure ulcers and found this facility was at roughly 1.5 times the national average in this metric.

In The Colony Healthcare Center, 11.96% of Patients use Anti-Psychotic Medication

This is the percent of patients who were prescribed antipsychotic drugs.
