Facility Inspections

Grade: B-minus

Although we gave this facility a terrible overall grade, we awarded it a B- for our inspections rating. In fact, the facility received very impressive inspection report this year. Inspection scores weigh several factors, including deficiencies, substantiated complaints and federal fines. You can learn more about each of these items by reviewing copies of nursing homes' inspection reports. This facility was assessed 7 deficiencies on its inspection report, but none of the deficiencies were considered to be major deficiencies. This means that CMS didn't consider any of the deficiencies an imminent risk to resident safety or health. A few minor deficiencies aren't necessarily the end of the world.

Long-term Care Quality

Grade: C

We were also stunned to find that this nursing home received a somewhat respectable long-term care rating this year. We gave it a C in this area. In a long-term care setting, the nursing home's primary objective is to keep patients healthy and safe. This is different than skilled nursing or short-term care where the goal is to rehabilitate patients. In addition to assessing the volume of care provided by aids and other staff, we also looked at the number of residents vaccinated for pneumonia. This nursing home provided the vaccine to 86.10039 percent of its residents. This is pretty significantly lower than what we expected. On a more positive note, this place was able to limit hospitalizations. It had only 0.01 hospitalizations per 1,000 long-term resident days, which is a remarkably low number.

Nurse Quality

Grade: D

This facility's next best category was nursing, but even there it received a grade of just D. This is still a well below average grade. The nursing grade weighs a handful of components, but the primary one is the level of nurse hours per patient per week. With just 0.3 hours of nursing care per patient on a daily basis, this nursing home's nurse staffing levels were significantly below average. Finally, we also assessed some nursing quality-based metrics in determining our nursing scores. This nursing home fared well in terms of avoiding pressure ulcers and major falls. Many consider these statistics to be good indicators of the quality of nursing care.

Short-term Care Quality

Grade: F

The last area we assessed is short-term care. This nursing home was given a poor grade of F in this category. In computing our short-term care ratings, we scrutinize a nursing home's levels of skilled nursing, which includes registered nurses, physical therapists respiratory therapists and other licensed professionals. This category is frequently a meaningful measure of the nursing home's rehabilitation. Considering its poor rating in this category, we were not shocked to discover that this place offered significantly fewer registered nurse and physical therapist hours per resident than most facilities. Lastly, we assessed the percentage of patients that returned home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 4.5 percent of this facility's patients were able to return home.
