Facility Inspections

Grade: D

In addition to receiving a poor overall score, we also gave this facility poor grades across the board in the various categories. In fact, it only received a grade of just D for its inspections grade, which is its best area. Our inspection grades factor in many items on the facility's recent inspection reports. When you find too many deficiencies on the inspection reports, this is typically the worst sign. Severe deficiencies indicate a threat to the well being of residents. This nursing home was assessed 3 deficiencies by CMS. The only favorable thing we found is that none of the deficiencies were in the categories that suggest that they posed a threat to patient safety or health. Finally, we need to draw your attention to the fact that this facility received large government fines in recent years in excess of $50,000. This typically indicates that inspectors grew tired after several years of failed government inspections.

Short-term Care Quality

Grade: D

Moving on to the area of short-term care, this nursing home was awarded a grade of just D in that area. Sadly, this wound up being one of its better features. In crafting these short-term care grades, we analyze the nursing home's skilled nursing services, such as those performed by registered nurses, respiratory therapists, physical therapists and other types of skilled professionals. The goal is to devise a scale for comparing the rehabilitation services of nursing homes. With a score in this range for this category, we weren't surprised to learn this place was below average in many staffing figures we looked at. The facility offered significantly fewer physical therapist and registered nurse hours per resident than most facilities. The last measure we looked at in this area is the number of residents who who were able to eventually return home from the facility. This nursing home struggled quite a bit in this area as well, with just 0 percent of its patients returning home.

Long-term Care Quality

Grade: F

The third area we analyzed was long-term care, in which this facility received an F. If you are seeking services other than short-term rehabilitation, you should take a close look at long-term care ratings. Once we concluded our assessment of the volume of nursing care, we then looked at the nursing home's vaccination record. We were optimistic to find that this nursing home vaccinated 95.89041 percent of its residents against pneumonia. To our surprise, this place was able to keep its patients out of the hospital. In fact, it had only 0.01 hospitalizations per 1,000 long-term resident days. Unfortunately, some of its other scores in the category weren't as favorable as these.

Nurse Quality

Grade: F

The last category we analyzed was nursing. This facility was given an abysmal grade of F in that area. When determining our nursing scores, we weigh both staffing levels and the levels of licensure of the nurses. This facility provided only 0.3 hours of nursing care per patient daily. This is an alarmingly low total. To go along with its below average totals in the area of nursing hours per resident, this facility had really bad marks in many of the quality-based metrics we looked at in determining our nursing grades. For starters, we looked at the percent of patients experiencing falls leading to major injury. This facility was at more than 150 percent of the national average in this statistic. This is alarming in light of the fact that many falls could be prevented with better nursing care. This ended up sealing the facility's fate of earning a poor nursing rating.
