Facility Inspections

Grade: A-plus

The primary reason this facility received a decent overall score is its impeccable health inspections in recent years. We awarded it one of our higher grades in that category, with a grade of A+. This is a much better grade than the facility's overall score, which was decent but certainly not elite. Arguably the most critical factor we consider in calculating our inspection scores is deficiencies. Deficiencies are found on a nursing home's inspection reports. Places with better scores in this category typically avoided the more severe deficiencies involving health risks or even death. Amazingly, this was one of the few nursing homes in the country that had no deficiencies whatsoever on its inspection report. This is very impressive. This is definitely a great sign for this nursing home.

Long-term Care Quality

Grade: C

We also awarded this facility a middle of the road score in the category of long-term care, where we gave it a grade of C. In forming these long-term care ratings, we quantify the facility's personal care services. One of the datapoints we considered in addition to nurse's aid hours was vaccinations. Thankfully, this nursing home administered the pneumonia vaccination to 96.34146 percent of its residents. This place was able to keep its patients out of the hospital. Indeed, it had only 0.01 hospitalizations per 1,000 long-term resident days, which is a remarkably low figure.

Short-term Care Quality

Grade: D

Moving on to the area of short-term care, this nursing home received just a D in this category. In our short-term care grade, we attempt to craft a valuable barometer for rehabilitation. In doing so, we look at a nursing home's levels of skilled nursing services, including both registered nurses and physical therapy, as well as speech therapy. With a grade of this caliber in this category, we weren't surprised to discover that this place was well below average in the two key staffing metrics we assess. The nursing home offered substantially fewer registered nurse and physical therapist hours per resident than most nursing homes. Lastly, we considered the number of residents that ultimately were able to return home from this nursing home. This nursing home didn't fare well here either. In fact, we found that just 4.9 percent of this facility's patients were able to return home.

Nurse Quality

Grade: D

This nursing home's weakest category was nursing, which is the last category we analyzed. For this category, we awarded this nursing home a D. When calculating our nursing grades, we weigh both staffing levels and the training levels of those nurses. This place averaged just 0.3 hours of nursing care per patient daily. This figure is quite a bit below the national average. Lastly, we also looked at some quality measures in calculating our nursing ratings. This nursing home was above average in two of the metrics we focus on, with solid marks for minimizing its residents' major falls and pressure ulcers. These data points are usually reliable measures of the quality of nursing care offered.

Heritage of Hudson Quality Metrics

Minimizes Pressure Ulcers

Grade: B-plus

In Heritage of Hudson, 6.93% of Patients had Pressure Ulcers

Minimizes Serious Falls

Grade: B-plus

In Heritage of Hudson, 2.44% of Patients had Serious Falls

This tells you the percentage of residents that have had a major fall. Falls leading to severe injury are considered by many experts to be an indicator of nursing care at a nursing home. Falls leading to injury are routinely the result of lower levels of patient supervision.

Minimizes Urinary Tract Infections

Grade: B-plus

In Heritage of Hudson, 1.23% of Patients had UTIs

This tells you the percent of long-term patients that have experienced a UTI. UTI's may be a sign of lower quality nursing care. However, this statistic may be misleading for some nursing homes due to different reporting standards for infections.

Grade: A-minus

In Heritage of Hudson, 9.91% of Patients use Anti-Psychotic Medication

This is the percentage of patients prescribed antipsychotic medications. While antipsychotic drugs may be helpful for many patients, it is important to make sure these medications are being used only where medically required. In limited cases, excessive reliance on these medications may suggest that a facility is using these drugs to subdue residents.

In Heritage of Hudson, 21.32% of Patients use Anti-Anxiety Medication

This indicates the percentage of patients who were given antianxiety drugs. These drugs are generally prescribed to patients experiencing depression or anxiety.

This datapoint is a measure of the number of rehospitalizations per thousand days of short-term care. Avoiding the hospital during rehabilitation is important to restoring the health of residents.

Short-term Care: ER Visits

Grade: B

In Heritage of Hudson, 11.26% of Patients

This is the number of emergency room visits per thousand days of short-term care. There is generally a correlation between reduced emergency room visits and the quality of nursing home care.

Short-term Care: Facilitates Functional Improvement

Grade: A-minus
