We want to draw your attention to the fact that this facility outperformed its overall score in the category of inspections. In fact, it received an excellent government inspection report this year. As a result, it received one of our highest grades in this category with an A+. Perhaps the most important factor we look at in calculating our inspection scores is deficiencies. These deficiencies are found on a facility's inspection reports. Nursing homes with better scores in this category typically dodged the more severe deficiencies involving health risks or even death. This nursing home was hit with 2 deficiencies on its inspection report, but fortunately none of the deficiencies were considered to be severe deficiencies. This indicates that CMS didn't deem any of the deficiencies an imminent risk to patient health or safety. A few minor deficiencies shouldn't stop you from considering a nursing home.

Long-term Care Quality

Grade: B-minus

We also gave this facility a middle of the pack score in the category of long-term care, where it received a grade of B-. In a long-term care setting, the primary objective is to maintain residents' quality of life and keep them safe. On top of considering the quantity of care provided by nurses aids and other staff, we looked at the number of residents vaccinated for pneumonia. Thankfully, this nursing home vaccinated 100 percent of its residents, which is a very respectable figure. This nursing home keeps its patients out of the hospital. In fact, it had only 0.01 hospitalizations per 1,000 long-term resident days, which is an impressively low figure.

Short-term Care Quality

Grade: F

Additionally, we gave this facility a rock-bottom F in the area of short-term care. Short-term care grades are meaningful for prospective residents needing rehabilitation. Rehabilitation typically requires additional highly-skilled nursing. This includes a vast spectrum of services, ranging from registered nurses to physical and speech therapists, as well as other types of therapy. With a grade this poor for this category, we were not shocked to find this facility was well below average in two of the key staffing metrics we looked at. The nursing home supplied far fewer physical therapist and registered nurse hours per patient than most other nursing homes. The final measure we looked at in this area is the percentage of patients who were able to leave the nursing home and return to the community. We discovered that just 5 percent of this facility's residents were able to return home, which is well below average. The combination of these subpar statistics doomed this nursing home's short-term care score.

Nurse Quality

Grade: F

This nursing home's weakest category was nursing, which is the last area we scored. Unfortunately, we gave it an F in this area. Even with a few decent grades in other categories, this grade is nevertheless of a concern so we would like to to draw your attention to it. Our nursing score looks at the facility's nurse staffing levels. We look at both the levels of skill of the nurses and the number of hours spent with patients. This facility provided only 0.1 hours of nursing care per resident on a daily basis. This is an alarmingly low total. To go along with its low total nursing hours per resident, this nursing home had really bad marks in several of the quality-based metrics we looked at in calculating our nursing grades. We looked at the percent of patients sustaining falls which led to major injury. This nursing home was at more than 150 percent of the national average in this statistic. This is quite concerning in light of the fact that so many falls could likely be prevented with better nursing care. This ended up sealing the facility's fate of receiving a poor nursing score.
