Facility Inspections

Grade: A-minus

While we were not high on this nursing home's overall grade, it received a very impressive inspection report this year. In fact, it received a nearly flawless inspection report this year. As a result, it received one of our best grades in that category with an A-. Our inspection ratings account for a host of factors found on a facility's inspection reports. One of the most important criteria we consider is the quantity and severity of deficiencies. You generally want to avoid nursing homes with a list of severe deficiencies flagged. This nursing home received 3 deficiencies on its inspection report, but fortunately none of the deficiencies were considered severe. This tells you that CMS did not deem any of the deficiencies to pose an immediate threat to patient health or safety. A couple minor deficiencies are not necessarily the end of the world.

Long-term Care Quality

Grade: D

We gave this nursing home just a D for our long-term care rating and that actually ended up being its next most favorable category. When facilities receive a score in this range in this category it typically means the nursing home did not perform well in our measures relating to patient care. On top of considering the amount of care provided by nurses and other staff, we looked at the percentage of residents vaccinated for pneumonia. This facility vaccinated 73.40067 percent of its patients. This is significantly less than what we expected. Surprisingly, this nursing home was actually able to limit hospitalizations. With just 0.01 hospitalizations per one thousand long-term resident days, this facility had fewer hospitalizations than the average nursing home. This is its best feature in this category.

Nurse Quality

Grade: F

In addition, we gave this facility an F in the area of nursing care. Our nursing rating is based on a handful of subcategories, however, the primary consideration is the quantity of nurse hours per patient per week. This facility provided just 0.3 hours of nursing care per resident per day. This is a very low total compared to most nursing homes. Finally, we also looked at some nursing quality-based metrics in computing our nursing grades. This place fared well when it comes to minimizing its residents' major falls and pressure ulcers.

Short-term Care Quality

Grade: F

The final category we graded was short-term care. We gave it a lowly F for this category, which is a bottom of the barrel score. In our short-term care assessment, we endeavor to forge a fair barometer for rehabilitation. In doing so, we analyze the facility's scope of highly-skilled nursing services, including both registered nurses and physical therapy, as well as respiratory therapy. As you might expect, we learned that this place provides far less registered nurse and physical therapist hours per resident than a typical nursing home. The last metric we looked at in this area is the percentage of patients who were able to leave the facility and return to the community. We discovered that just 3.6 percent of this facility's residents were able to return home, which is well below average. The combination of these subpar metrics doomed this facility's short-term care grade.
