Facility Inspections

Grade: F

This place's grades were terrible across the board. In particular, we wanted to point out its inspection grade where it received an F. This isn't a good sign for its inspection reports. With facilities that received this bad of an inspection score, we recommend scrutinizing any severe deficiencies on its inspection report. This particular facility was assessed 9 deficiencies by CMS. The only positive thing we found is that none of its deficiencies were in the categories that suggest they posed a risk to resident safety or health. Finally, this facility was cited by CMS for possible abuse or neglect. We would not recommend this facility under any circumstance.

Nurse Quality

Grade: F

Unfortunately, we gave this facility an F for its nursing grade. Our nursing score is based on the facility's nurse staffing levels. We factor in both the levels of training of the nurses and the quantity of hours spent with patients. This facility provided only 0.2 hours of nursing care per resident per day. This is well below average. Finally, we also looked at some nursing quality-based metrics in calculating our nursing ratings. This facility performed well in the area of minimizing its residents' pressure ulcers and major falls. We consider these statistics to be reliable measures of the quality of nursing care.

Long-term Care Quality

Grade: F

This nursing home wound up receiving a rock-bottom score in our long-term care category. It received a lowly F in this area. Facilities that receive this type of score in this category may not provide the type of consistent 24/7 care that some other facilities provide. On top of assessing the quantity of care provided by nurses and other staff, we analyzed the percentage of residents vaccinated against pneumonia. This facility gave the vaccine to 30.757801 percent of its patients. This is significantly lower than the national average. Surprisingly, this facility actually fared well at keeping its patients out of the hospital. It had just 0.03 hospitalizations per one thousand long-term resident days. This was its best feature in this category.

Short-term Care Quality

Grade: F

The last category we rated was short-term care, where this facility was given a poor score in this category also. Unfortunately, we gave it an F in this area, which is obviously very concerning. In our short-term care assessment, we attempt to create a meaningful measure for rehabilitation. In doing so, we assess the facility's levels of highly-skilled nursing services, including both registered nurses and physical therapy, as well as occupational therapy. With its grade in this area, we were not stunned to learn that this place was well below average in terms of its quantity of physical therapy and registered nurse hours offered to its patients based on the data we assessed. Frankly, this was about what we expected here. The final datapoint we considered in this area is the number of patients who were able to leave the facility and return to the community. We discovered that just 3.1 percent of this facility's residents were able to return home, which is well below average. The combination of these subpar metrics doomed this nursing home's short-term care score.
